Insurance: 

Insurance: I am an in-network provider for Blue Cross PPO plans (but you should always check on this since there are so many plan variations. 

 

For other plans, as an out of network provider, I will fill out the necessary paperwork for you to be reimbursed by your insurance provider if that is possible.

 

 

Fee:

$200/session; I do offer some lower fees, particularly for college students.

Below are three forms. Please fill out the Personal Information form prior to the appointment if possible. You will find useful details about my practice in About My Practice. Last, you will need to sign off that you have read the HIPAA form which is on the website and in hard copy at the office.
